Hours: Pool - As Needed; Mon-Fri 8-430p to cover for time off, leave of absence, or high census days. Commit to availability of 2 shifts per month.
Full Time / Part Time: Pool/As Needed - no Benefits
The Speech Pathologist works collaboratively with patients, nurses, social workers, physicians, other practitioners, caregivers and community agencies to coordinate the patient plan of care for speech and language pathology services in the home care setting for adults 18 and over. Involves support systems in care. Involves patient in decision-making. Adapts for sensory losses (i.e. hearing, sight, temperature sensation, taste). Collects client health data, prioritized by the client’s condition or needs, utilizing appropriate techniques while involving the client, family, and other health care providers, as indicated. Develops a plan of care, based upon identified problems, prescribes interventions to attain expected outcomes. Responsible for the client’s appropriate and uninterrupted care throughout the health care continuum, by coordinating services, discharge planning, and community resources.
- Masters in Speech Pathology
- 2 Years’ Experience in a Home Health Setting Preferred
- Speech Language Pathologist, WI License
- BLS (CPR)
- Valid Wisconsin Driver's License
